You can print, fill out and bring our Registration Form, in order to speed up registration at your first visit.

Be on time.
When you arrive on time for your appointment, we will honor your appointment time. A patient, who arrives late, would have to wait if the next patient came on time.

Cancellation.
If you need to cancel a scheduled appointment, please let us know in advance, and we will schedule your new appointment as soon as possible.

Change of information.
If you have made a change in your personal information, please let us know. For changes such as address, phone number, employment and insurance provider, you can call, e-mail, or inform us during your visit to the office.
